Recording the outcome of appointments
admin outcome
The outcome of an appointment is made up of two parts.

The clinical outcome
This is recorded by a clinician during and after an appointment.

The administrative outcome
This is recorded by an office administrator after a clinician has finished recording the clinical outcome.
If the patient doesn't attend the appointment, or the appointment is cancelled, you only record the administrative outcome for the appointment.
When you record the administrative outcome of an appointment, you can:

Book a follow-up appointment

Add the patient to a waiting list

Put the patient on a PIFU Patient Initiated Follow-Up. Allows patients and their carers the flexibility to arrange their follow-up appointments as and when they need them. pathway

Discharge the patient from your care

Refer the patient to another organisation
The outcomes available for you to choose depend on your organisation.
Recording the outcome of appointments on a mobile
You can't do this on a mobile.
Recording the outcome of appointments on a desktop
Recording the clinical outcome of an appointment

A clinician completes the clinical outcome form.
They can do this during and after the appointment.
Progress is saved automatically.

Optional: transcription.
-
The clinician sends the clinical outcome form for transcription

A member of staff transcribes clinical notes that contain audio
-
The clinician reviews the transcription before submitting the form

The clinician sends letters about the appointment to relevant people.
Nervecentre uses the information in the form to generate these letters.

Recording the administrative outcome of an appointment

Select Outpatients > Clinic List > double-click on a patient's name > Patient Detail.
Depending on your permissions, you might need to select Go to patient record.

From Patient Detail > Patient admin > Show pathways > Appts, find an appointment with one of the following statuses:

'DNA, outcome required'
-
'Clinic complete, admin outcome required'
-
'Cancelled, outcome required'

Select Outcome appointment.
A form appears. This shows:

The current RTT Referral To Treatment. The length of time from referral to treatment. status of the referral

Any booked future activities related to the referral
-
The clinical outcome of the appointment
-
Any procedures related to the referral
-

If the patient attended the appointment, select if treatment commenced.

From Outcome, select an outcome.
More options appear. These options depend on the outcome you chose.
The outcomes available depend on your organisation.

Select options until Continue appears.

Select Continue.
What appears next depends on the options you chose.
One of the following appears:

Future Activity
You must decide what happens to other activities related to the referral. For example, internal referrals and waiting list entries.

Actions Taken / Required
Shows any actions Nervecentre has automatically done. For example, added the patient to a PIFU list or waiting list.
Shows any actions you need to do before you can record the outcome. For example, list a procedure, or book an appointment.

No Actions Required
Appears when you don't need to anything other than record the outcome.

Select Complete Outcome.

Optional: depending on the outcome you selected, you can send letters about that outcome.
For example, you might need to send letters when you place the patient on a PIFU pathway, or book a new appointment.

Select a correspondence option to change who the letters are sent to.
To not send the letters, select None.
The options you see depend on your organisation.

Select Click to preview letters.
The letters appear in your organisation's PDF viewer.
A list of letters appears on the right. This shows the recipient of each letter and how the letter is delivered. For example, 'GP - MESH' or 'Patient - Manual print'. To view a letter, select view.
You can also see information about the patient's preferred method of receiving letters, and preferred letter format.

Depending on your permissions, if Nervecentre is sending a patient's letter by the patient's preferred delivery method, by Docmail, or by email, you can choose to manually print and send the letter instead.
To manually print and send a patient's letter, select Prevent sending of this letter, I will manually print it instead.

Select Send.
The appointment moves to Past and Cancelled Appointments and the appointment's status updates.

Recording the administrative outcome of an appointment retrospectively
You can record the administrative outcome of an appointment retrospectively. This allows you to set the outcome and RTT Referral To Treatment. The length of time from referral to treatment. code without booking further activity, or affecting the RTT pathway. For example, if you record the outcome of the follow-up appointment before the initial appointment, you can record the outcome of the initial appointment retrospectively.

Select Outpatients > Clinic List > double-click on a patient's name > Patient Detail.
Depending on your permissions, you might need to select Go to patient record.

From Patient Detail > Patient admin > Show pathways > Appts, find an appointment with the status 'Clinic complete, admin outcome required'.

Select Outcome retrospectively.
A form appears. This shows:

The current RTT status of the referral

Any booked future activities related to the referral

The clinical outcome of the appointment

Any procedures related to the referral

If the patient attended the appointment, select if treatment commenced.

From Outcome, select an outcome.
More options appear. These options depend on the outcome you chose.
The outcomes available depend on your organisation.

Select options until Continue appears.

Select Continue
No Actions Required displays to indicate you don't need to do anything else.

Select Complete Outcome.
The appointment moves to Past and Cancelled Appointments, and the appointment's status updates.
Changing the outcome of an appointment
If you recorded the outcome of an appointment, but an error was made, you can change the outcome of the appointment. This changes any actions taken as a result of the previous outcome.
For example, the outcome of an appointment was to book a follow-up appointment, but instead, you discharged the patient in error. When you discharged the patient, Nervecentre closed the referral. You can undo the outcome of the appointment, and when you do that, Nervecentre reopens the referral. You can then record the intended outcome of the appointment and book a follow-up appointment.
If you don't want Nervecentre to undo or change previous actions, depending on your permissions, you can edit the outcome of an appointment.

To change the outcome of an appointment:

Select Outpatients > Clinic List > double-click on a patient's name > Patient Detail.
Depending on your permissions, you might need to select Go to patient record.

From Patient Detail > Patient admin > Show pathways > Appts > Past and Cancelled Appointments, select an appointment.

Select Undo Outcome.
A dialogue box appears that lists the changes that occur when you remove the existing outcome. For example, the episode reopens, the referral reopens, and the RTT Referral To Treatment. The length of time from referral to treatment. status changes back to its previous state.

Select Undo.
The appointment moves to Booked Future Appointments.
If the previous outcome for the appointment was to book a follow-up appointment, when you undo the outcome of the appointment, the follow-up appointment goes to the status 'Booking in progress', and you need to delete that follow-up appointment.
Editing the outcome of an appointment
Depending on your permissions, you can edit the outcome of an appointment. This allows you to record the correct outcome for data quality purposes. When you edit an outcome, Nervecentre doesn't change or undo any actions taken for the previous outcome.
For example, you moved your appointments from a third-party system to Nervecentre. Nervecentre doesn't have all the outcome details for those appointments, so you can't undo the outcome, but you can edit the outcome.
We recommend that you change the outcome of an appointment with Undo Outcome when it is possible.

To edit the outcome of an appointment:

Select Outpatients > Clinic List > double-click on a patient's name > Patient Detail.
Depending on your permissions, you might need to select Go to patient record.

From Patient Detail > Patient admin > Show pathways > Appts > Past and Cancelled Appointments, select next to an appointment.

Select Edit Outcome.

Select a new outcome for the appointment.

Select Save.
