Paper case notes, Physical case notes
You can manage and track paper case notes from Medical Records.
In Nervecentre, you can view a list of case note volumes A physical folder that contains paper case notes. These paper case notes contain medical information about a patient, for example, what happened during a visit to a hospital or an outpatient appointment. When a case note volume is full, a new case note volume is created. for a patient from:

Medical Records > Manage Case Notes, select Case Notes for Patient

Patient Detail > Patient admin > Manage case notes

About medical records on a mobile

About medical records on a desktop

A case note volume is a physical folder that contains paper case notes. These paper case notes contain medical information about a patient, for example, what happened during a visit to a hospital or an outpatient appointment. When a case note volume is full, a new case note volume is created.
In Nervecentre, you can view a list of case note volumes for a patient from:

Medical Records > Manage Case Notes, select Case Notes for Patient

Patient Detail > Patient admin > Manage case notes

Nervecentre also has digital case notes for patients. You can view a patient's digital case notes from Patient Detail > Case Notes.

From Medical Records, you can manage and track paper cases notes.

As a member of staff, you can:

View loan requests
For example:

A clinician might request a case note volume when a patient has an appointment or an inpatient admission.

A clinical coder might request a case note volume when doing clinical coding for a patient.

An office admin might request a case note volume if a patient makes a subject access request.

As a medical records manager, you can:

Create and manage pull lists

Assign pull lists to medical records clerks

Create reports

As a medical records clerk, you can:

View and complete your pull lists

Record the location of multiple case note volumes

Before a case note volume can be requested:
-
A medical records manager creates pull lists
-
A staff member creates a physical case note volume for the patient
They add the case note volume to Nervecentre and print a label for the case note volume.

A staff member requests a case note volume about a specific patient.
-
Nervecentre places the loan request on a pull list.
-
A medical records manager assigns the pull list to a medical records clerk.
-
A medical records clerk completes their pull list.
-
They view their assigned pull lists.
-
They print a list of loan requests on their pull list.
-
They find the case note volume that has been requested.
-
They print a tracking label for the case note volume.

The clerk takes the case note volume to the library, and records its location.

The staff member uses the case note volume.

A staff member marks a case note volume as 'Return to library'. The case note volume has the borrower code 'RTL'.

Nervecentre places the loan request on a pull list.

A medical records manager assigns the pull list to a medical records clerk.

A medical records clerk completes their pull list.

They view their assigned pull lists.

They print a list of requests on their pull list.

They find the case note volume that has been requested.
