Viewing assessment charts

You can view the information added into an assessment in a chart. The chart provides you with the latest information from assessments. You can view previous assessments, including assessments recorded in a previous episode of care.

Viewing assessment charts on a mobile
  1. From your patient list, select a patient.

  2. Select Assmnts.

  3. Select the name of the assessment you want to view.

    To search for an assessment, select and enter the name of an assessment.

  4. Select Chart.

Shows the patient's demographics.

The default view is the assessment you selected. Select an option to choose the assessments to navigate through.

Select All to navigate through all assessments.

When you select a specific assessment, more details about that assessment appear under the summary of the assessment.

Select and to view started and completed assessments.

Shows a summary of the assessment.

If the summary has a grey background, the assessment has been struck out.

Type

The name of the assessment.

Result

The outcome of the assessment, for example, 'High Risk. Needs Treatment' or a RAG rating.

Score

Any severity score associated with the outcome of the assessment.

Status

The status of the assessment.

Due

If an assessment must be completed before a certain date, the date and time are shown here.

Ward

The location where the assessment was completed.

By

The user who completed the assessment. Show the user's full name and username.

Comment

Shows any information added by users, for example, if the assessment is struck out.

Contributors

If an assessment is countersigned, this is shown.

Shows the name of the user who completed the assessment and the name of the user who amended the assessment before countersigning it.

Shows the date and time the assessment was started.

Select a date and time to strike out an assessment.

Viewing assessment charts on a desktop

  1. Open the patient's record.

    • From  Inpatients, select Patient List > double-click on a patient's name > Patient Detail.

    • From  Outpatients, select Clinic List > double-click on a patient's name > Patient Detail.

  2. From Clinical, find Assessments.

  3. Select .

Shows the patient's demographics.

The default view is 'All'.

Select an option to choose the assessments you can navigate through.
