Managing escalations when you submit observations
When you record an observation, escalations might be triggered. An escalation is a set of observations sent directly to the mobile device of a member of staff trained to respond to an unwell patient.
You can manage escalations after you submit an observation.

Before you submit observations and escalations you can:

Choose to not inform someone about a raised or high EWS Early Warning Score. A vital signs recording and scoring system used with adult patients. The scoring system is designed to identify and alert appropriate staff in the case of deteriorating patients..
You must give a reason for not informing someone.

Choose to inform additional clinicians.

Add notes, which are included in the escalation message.

The scoring system is designed to identify and alert appropriate staff in the case of deteriorating patients. when observations are recorded, Nervecentre might decide to send an escalation to another clinician.
To choose not to inform a specific role of a default escalation:

From the default escalation options, select next to the user role.
Active options appear green on iOS and blue on Android.

Select Submit Observation.

When you choose to not send to a default escalation, you must give a reason. Select from one of the options.
The default escalation options depend on your location, and the time of day.

Some observations have an alternate escalation available. These are members of staff that are not part of the default escalation, but you might want to let them know. For example, CCOT (Critical Care Outreach Team).
To choose an alternate escalation:

From the alternate escalation options, select next to the user role.
Active options appear green on iOS and blue on Android.

Select Submit Observation.
The alternate escalation options depend on your location, and the time of day.

You can add notes in the submission page. These notes are added into the chart and the escalation message is sent.
To add a note:

From Notes, select Background/History.

Enter a note.

Select Add to save the note.

Select Cancel to go back.

Select Submit Observation.

Some users, for example, healthcare assistants or student nurses, do not have permission to submit escalations. Users with permission to enter observations can submit observations with no associated escalations. If the observations have associated escalations, they must be authorised by a user with permission to authorise escalations.
Depending on your permissions, you can authorise escalations.
To authorise an escalation:

Select

Enter your username and password.

Select OK.

Edit any escalations as needed.

Select Submit Observation.

Choose No to change from the default escalation.

Select Submit Observation.

After entering an observation, the Alerts and Escalations page appears. You can add notes in the Alerts and Escalations page. These notes are added into the chart and the escalation message is sent.

From Nursing Notes, select Add comment....

Enter a note.

Select Save to save the note.

Select Cancel to go back.
