Alternative observation models and charts
Patients have a base observation model applied to their record. Depending on your permissions, you can:

Change the base model
You can change the base model to apply different observation parameters, pop-up alerts and escalations. For example, when the patient is receiving a blood transfusion.

Add a second model.
This model is a governed chart with its own rules and escalations. For example, a weekly height and weight chart.

Record an additional observation.
This might be an interim observation, for example, temperature or recording bowels. Recording these does not impact the EWS Early Warning Score. A vital signs recording and scoring system used with adult patients. The scoring system is designed to identify and alert appropriate staff in the case of deteriorating patients. frequency.
Alternative observation models and charts on a mobile

There are several options available to you in recording different observations for your patients:

Special Circumstances.
Special circumstances change the base model. You might see different observations, pop-up messages, and escalations.
For example, you can use special circumstances to change the base model from NEWS2 to NEWS2 scale 2 for patients with a hypercapnic respiratory drive. NEWS2 scale 2 has a lower threshold for oxygen saturation levels.
There are other models available, including end of life care, and neurological observations.

Governed Charts
You can add a second base model called a governed chart. Governed charts have their own frequency, pop-up messages, observations, and escalations. Adding a governed chart does not change the original base model.
For example, you can add a governed chart for patients who need mental health observations every hour, in addition to NEWS2 every 6 hours.

Additional Charts
You can add additional charts to record an observation that does not need to be regularly recorded.
For example, you could add an additional chart for bowels, or interim observations. An interim observation is an observation included in the base model, but recorded in-between the usual readings set by the EWS frequency. For example, an oxygen saturation or a temperature.

From your patient list, select a patient.

From Obs & Assessments, select Obs.

Select Set Special Circumstances.

Select a circumstance from the list.
You can search for a circumstance.
When you select a circumstance, you are shown sn explanation of what the model is used for.

Select an option.

Select Set Circumstances to set the circumstances.
Or

Select Cancel to go back.
The new chart appears and the history records the changed circumstances.
If configured, obs due alerts are sent when the model is changed.

You might only be able to set one governed chart. Governed charts are configured in families. You can only add one governed chart from a specific family. For example, one mental health governed chart and one pain chart.

From your patient list, select a patient.

From Obs & Assessments, select Obs.

Select Add Governed Chart.

Select a governed chart.
You can search for a governed chart.

Select an option.

Select Add Chart to add the governed chart.

Select Cancel to go back.
If configured, obs due alerts are sent when the model is changed.

Select the chart name, record and submit the observations.

From your patient list, select a patient.

From Obs & Assessments, select Obs.

Select and hold on the name of the governed chart.

Select Remove Chart.
You can’t undo removing a chart. If you removed the chart in error, add the chart again.

From your patient list, select a patient.

Select Additional Charts.

Select an option to record observations.

Record and submit the observations.

Sometimes you might need to return a patient to the original base observation model. For example, when a blood transfusion is complete.

From your patient list, select a patient.

From Obs & Assessments, select Obs.

Select Revert to Default.
-
Select Yes to dismiss the warning.
The patient is returned to the default chart, and the history records the changed circumstances.
Alternative observation models and charts on a desktop
You can add special circumstance observation models and charts on a desktop, but you can't add governed charts or additional charts. You can return patients to their default chart.
You can add additional observations on a desktop, if the observations are in a form that also contains the specific additional chart.

From Inpatients > Patient List > double-click on a patient's name > Patient Detail, select Clinical.

From Observations > Chart, select the name of the chart.

Select Set Special Circumstances....

Select the special circumstance.

You can only add additional observations on the desktop when the form contains the specific additional chart. You can’t choose an additional chart on the desktop.

Sometimes you might need to return a patient to the original base observation model. For example, when a blood transfusion is complete.

From Inpatients > Patient List > double-click on a patient's name > Patient Detail, select Clinical.

From Observations > Chart, select the name of the chart.

Select Revert to Default.
