Confirming retrospectively that a dose was administered

1. From your patient list, select a patient.

2. From Chart, select a prescription.

3. Select a dose and then select Confirm given retrospectively.

   

4. Select Time given and select a time and date.

5. To add a note, select Notes and enter a comment

Part 1, to be completed by the medication administrator

Complete this task from your own account.

1. From your patient list, select a patient.

2. From Chart, select a prescription.

3. Select the dose.

4. Select Confirm given retrospectively.

5. Complete required fields.

6. Select Submit.

Part 2, to be completed by the witness

Complete this task from your own account.

1. From your patient list, select a patient.

2. From Chart, select a prescription.

3. Select the dose.

   

4. Select Record witness, enter a note, and select OK.

   

If you are unable to record a medication dose at the time of administration, you can retrospectively confirm that it was given.

1. From  Inpatients > Patient List > double-click on a patient's name > Patient Detail, select Meds.

2. From the MAR Medicines Administration Record. Also known as drug chart. A list of all medication prescribed and administered (given)., select a dose marked Due, or Overdue.

3. Select Confirm given retrospectively.

   For infusions, you can select Confirm started retrospectively or Confirm given retrospectively.

4. Complete any mandatory fields.

   Mandatory fields are marked .

   For example, the dose quantity, dose rate, route, or volume given.

5. Enter date and time the dose was administered.

   For infusions, enter the time started and/or stopped.

6. Add notes as needed.

   

7. Select Submit.

If you are unable to record a witnessed medication dose at the time of administration, you can retrospectively confirm that it was given and witnessed.

If a medication dose that needs a witness is given retrospectively,  appears in the dose cell on the MAR, with the recorded time of administration.

1. From  Inpatients > Patient List > double-click on a patient's name > Patient Detail, select Meds.

2. To witness the medication dose retrospectively, select the needed cell > Record witness.

3. Enter notes and save.

    no longer appears in the dose cell on the MAR.

   You can't strike out a retrospective witness signature.
