Streaming patients in UEC
When a patient attends UEC Urgent and Emergency Care. In Nervecentre, this covers 'ED areas' and SDEC., you can review the patient's condition to determine the care they need. This process is called streaming. When you review a patient's condition, you complete a streaming form. You might decide the patient needs to stay in an ED area 'Emergency department' areas. These areas can be in emergency departments, urgent treatment centres, urgent care centres, and minor injury units. These areas handle acute and non-acute medical emergencies, and they have a four hour target., go to SDEC Same Day Emergency Care. These areas aim to complete treating patients without a hospital admission or overnight stay. These areas don't have a four hour target., go visit their GP, or go home.
If the patient is being streamed away from an ED area, you must depart the patient from the ED area and perform any necessary administrative actions, for example, preadmitting the patient to SDEC.

From your patient list, select a patient.

Select Case Notes.

Select a case notes profile to filter the list of interventions.
For example, 'ED Current Episode'.

Select and choose an intervention for streaming.
The name of this form depends on your organisation.

If you see No form set, the intervention has a selection of forms you can choose from. Select the intervention and choose the form you want to use.

Select the intervention from the list to open the streaming form.

Complete the form, including all mandatory fields.

Mandatory fields are marked .

Some fields might already be populated with information added elsewhere in Nervecentre.

From the bottom of the form, select Submit.
You must select Submit at the end of the form to sign the form and capture the moment in time.
Streaming patients in UEC on a desktop

From Inpatients > Patient List > double-click on a patient's name > Patient Detail, select Case Notes.

From the top of the list on the left, select to choose a case notes profile.
The case notes profile changes the entries shown.
Depending on your permissions, you might be able to view all entries in a patient's case notes. To view all entries, select All case notes.

Select the patient's visit to an ED area 'Emergency department' areas. These areas can be in emergency departments, urgent treatment centres, urgent care centres, and minor injury units. These areas handle acute and non-acute medical emergencies, and they have a four hour target..
Visits to an ED area show and ED attendance.

How you add an intervention depends on the layout of the case notes profile.

To add an intervention:

To a collapsible tree:

Mouseover the patient's visit and select +.

Select the streaming form.
The form's name and location depends on your organisation.

To a fixed tree, or a flat layout:

From the top right of the list, select +.

Select the streaming form.
The form's name and location depends on your organisation.

If there is a single form associated with the intervention, the form opens on the right.
Or, if there are several forms associated with the intervention, from the top of the form, select Choose Form.

Complete the form including all mandatory fields.

Mandatory fields are marked .

Some fields might already be populated with information added elsewhere in Nervecentre.

Submit the form, or strike out the form.

To submit the form, select Submit.

To strike out the form, select Cancel.
This doesn't remove any entries you added to the form. You must remove these manually.
