Select next to the name of the clinical note.

Select Show History.

Select the entry.

Select Strikeout.

Enter a reason.

Select Strikeout.
Reverting a clinical note
If an entry has been struck out, you can revert the entry so it is no longer struck out.

Select next to the name of the clinical note.

Select Show History.

Select the entry.

Select Revert.
Clearing a clinical note
Depending on your permissions, you might be able to clear the value in a clinical note. Clearing a clinical note makes the value in the note blank. The previous values in the note are still shown in the clinical note's history.
If the clinical note has a workflow, clearing the clinical note can break the workflow. For example, the notes 'Sepsis Screening' and 'AKI' have a workflow.

Select next to the name of the clinical note.
