Nervecentre V7.1 Help for Users
Home leave and TTOs
Home leave describes when a patient is allowed to temporarily leave hospital for a period of time before returning. For example, a patient having procedures on Monday and Wednesday might be able to go home on Monday night and return Wednesday morning as part of planned care.
During this leave of absence, the patient might need to be supplied with regular medications, and any new medications.
The process for home leave will vary between organisations. Please ensure you familiarise yourself with local policies.
You cannot manage home leave on a mobile device.
Home leave and TTOs on a desktop
Home leave within Nervecentre works in a very similar way to discharge. You create a home leave letter and a TTO for home leave medications. The two steps can be completed in either order, or at the same time.

In case notes, complete a home leave letter. This is equivalent to the clinical discharge summary in a standard discharge.
Case Notes > > Home Leave, although it might have a different name for your organisation.
Complete all mandatory fields.

Complete the TTO To Take Out. Often used to describe the medicines added to the discharge letter, and, informally, the letter itself. for the period the patient will be at home.
When the TTO list is complete, scroll to the bottom of Case Notes and Approve / Send to Pharmacy. Alternatively, if permitted by your organisation's policy, you can select Approve, ward supplied TTO. This will bypass pharmacy, and means that there will not be a pharmacy check.
In pharmacy, medications are checked. From Case Notes or Pharmacy Pages, pharmacists see the following options:
The ward will receive any dispensed TTOs and give to the patient. Confirm ward received and verified.

You can print a 14-day administration chart for TTO medications.
-
From patient detail, select Meds > > TTO.
-
Select
.
-
Select TTO MAR Chart.
-
Download or print the PDF file.

When you have created the letter and TTO summary, electronically sign the home leave letter to the GP.

Electronically send the letter.
Prescriptions will remain active on the MAR Medicines Administration Record, also called drug chart: a list of all medications prescribed and administered.. You might need to pause these medications, or mark as ‘omit’, depending on your organisation’s policy.

From patient detail, select Meds > > TTO.
The TTO list is shown. Select Restart TTO to delete this list and start again for the patient’s next home leave or standard discharge.
