Decoupling a local patient record from a patient record on PDS

You can decouple a patient's local record from their record on PDS Personal Demographic Service, an NHS system. The PDS helps healthcare professionals to identify patients and match them to their health records. It also allows them to contact and communicate with patients.. You might decouple a patient's record from PDS if:

  • The local record is more up-to-date, and you can't update the record on PDS

  • The local record is linked to the incorrect record on PDS

  • The patient can't be matched to a specific record on PDS

  • The patient can't be found on PDS
