Marking a patient as deceased
You can mark a patient who is admitted as deceased by completing an assessment, or a form.
If a patient is not admitted, you can mark them as deceased from Patient Detail > Patient admin. You can also mark a patient as not deceased.
Marking a patient as deceased on a mobile
On a mobile, you can mark inpatients, and patients in UEC as deceased. You can't mark outpatients as deceased on a mobile.

- From your patient list, select a patient.

Select Assmnts.

Select + to start a new assessment.

Select an assessment for marking the patient as deceased.
The name of the assessment depends on your organisation.
To search for an assessment, select and enter the name of an assessment.

Complete the questions in the assessment.

To continue to the next page, select Next.

To go back to the previous page, select Prev.

You might have the option to save your answers before submitting the assessment.
To save your answers, select Save.

You might be shown a summary. The summary might show you a risk score and your organisation's policy on what to do next.
Select Next to continue to the next page.

Select Submit/Save.

If the assessment doesn't need countersigning, Submit/Save submits the assessment.

If the assessment does need countersigning, Submit/Save saves your responses. The clinician that countersigns the assessment can view your responses, then submit the assessment.

To return to the list of completed assessments:

On iOS, select Back.

On Android, select your mobile's back button.

Marking a patient as deceased on a desktop
On a desktop, you can mark inpatients, patients in UEC, or outpatients as deceased.

From Inpatients > Patient List > double-click on a patient's name > Patient Detail > Clinical, find Assessments.

Select +.

Select a folder of assessments from the list.

Select an assessment for marking the patient as deceased.
The name of the assessment depends on your organisation.

Complete the questions in the assessment.

To continue to the next page, select Next.

To go back to the previous page, select Prev.

You might have the option to save your answers before submitting the assessment. This allows you to do something else and return to the same point in the assessment later. Saving also prevents you losing progress if you accidentally click off the assessment.
To save your answers, select Save.

You might be shown a summary. The summary might show you a risk score and your organisation's policy on what to do next.
Select Next to continue to the next page.

Select Submit/Save.

If the assessment doesn't need countersigning, Submit/Save submits the assessment.

If the assessment does need countersigning, Submit/Save saves your responses. The clinician that countersigns the assessment can view your responses then submit the assessment.

If a patient is admitted, you can mark them as deceased by completing a form. For example, if your patient is an inpatient, or a patient in UEC, and they died during their visit.
- From Inpatients > Patient List > double-click on a patient's name > Patient Detail > Patient admin, select Discharge from ward or Depart from ED.

From the top left of the form, select .

Select a form that allows you to discharge the patient from their current visit, and admit them to the mortuary.
The name of this form depends on your organisation.

Complete the form, including all mandatory fields.
Mandatory fields are marked .

At the bottom of the form, select Discharge Transfer to Mortuary.
The text on this button depends on your organisation.

If a patient is not admitted, you can mark the patient as deceased in Nervecentre. For example, if your patient is an outpatient and they died before their appointment.

Open the patient's record.

From Inpatients > Patient Detail, search for the patient.

From Outpatients > Patient Detail, search for the patient.

From Patient admin, select Patient deceased.

Select Record patient as deceased.

Select the time of death.
-
Optional: enter any notes.

Select Set as deceased.

If you mark a patient as deceased in error, you can mark the patient as not deceased.

Open the patient's record.

From Inpatients > Patient Detail, search for the patient.

From Outpatients > Patient Detail, search for the patient.

From Patient admin, select Patient deceased.

Select Undecease patient.
-
Enter a reason for marking the patient as not deceased.

Select Undecease patient.
A warning message appears. The message explains that if you mark the patient as not deceased, Nervecentre reinstates all cancelled activities.
