Reconciling, pausing, and stopping arrival medications
You can create a hospital prescription from an arrival medication, reconcile an arrival medication with a hospital prescription, pause an arrival medication during the patient's stay, or stop an arrival medication.
When you stop an arrival medication, on discharge the patient's GP receives a letter saying the medication was stopped. Depending on the advice from the hospital, the patient's GP might start a new prescription.
When you pause an arrival medication the patient's original prescription continues to be valid. But the hospital might advise the patient and the patient's GP to stop the prescription.
When you reconcile an arrival medication with a hospital prescription, the patient's GP receives a letter with details of hospital care and the patient's original prescription continues to be valid.
You can't stop a paused arrival medication.

A hospital prescription is a current prescription issued while the patient is in hospital.
Reconciling, pausing, and stopping arrival medications on a mobile

To reconcile an arrival medication with a hospital prescription, you can either create a prescription from the arrival medication, or you can create the prescription first and then reconcile the arrival medication.

From Patient list, select a patient.

Select Chart.

Select .

Select Add from arrival meds.

Find and select the medication.

Prescribe the medication.
The arrival medication automatically reconciles with the new prescription.

To reconcile an arrival medication with a hospital prescription, you can either create a prescription from the arrival medication, or you can create the prescription first and then reconcile the arrival medication.
From Patient list > (select a patient) > Arr on > (select arrival med) > Reconcile > (select from current prescriptions) > Reconcile.

From Patient list > (select a patient) > Arr on > (select arrival med) > Reconcile, select either Pause for this visit or Stop.

From Patient list > (select a patient) > Patient Detail > Meds, select .

Select .

Select Add from arrival meds.

Prescribe the medication.
The arrival medication automatically reconciles with the new prescription.

To reconcile arrival medications, select > Arrived on.

This shows a menu of all current prescriptions. You can choose which prescription corresponds with the highlighted arrival medicine.

For each medication, select Reconcile, and then choose one of the options : Reconcile with current med, Pause drug for visit, or Stop drug.

To reconcile arrival medications with current medications, select > Arrived on.

Select Reconcile > Reconcile with current med.

This shows a menu of all current prescriptions. Select which prescription corresponds with the highlighted arrival medicine and add a reason.

If a medication is prescribed using Add from arrival meds the arrival medication will automatically reconcile with the current medication.

In Arrived on, comments appear under Notes.

To pause or stop arrival medications for the duration of the visit, select > Arrived on.

Select Reconcile and either Pause drug for visit or Stop drug.

Enter a reason. This will be included in the discharge letter.

Arrived on now shows the current state of the medication and any details.

Stopped medications can be reactivated when creating a TTO To Take Out. Often used to describe the medicines added to the discharge letter, and, informally, the letter itself..
Paused medications automatically resume on discharge.

Completing medicine reconciliation is permission based.

To complete the medicine reconciliation, select Reconciliation Complete and confirm.
To confirm all medicines have been added, select Med History Complete and confirm.

If further medications need to be added, or if the history is incomplete or incorrect, select Reopen arrival meds to edit.
